I’m not familiar with that idiom, and the context is ambiguous other than “something bad.”

New to me. Might be a regionalism, might be a Nina original.

At a guess, it’s also meant to suggest “hidden nastiness that may reveal itself later”, assuming that the ink and milk don’t mix.

… Maybe?

I mean, for that to be the case, the ink would have to be heavier than the milk as well, and it’d be a very odd idiom that would require knowing the miscibility and specific density of two fluids that usually don’t get mixed.

But that just might mean that Nina’s not good at inventing idioms.
