I think a big part of it is precisely because the media are sane-washing what Trump says. They paraphrase what he says in ways that it becomes coherent, or just talk about their interpretation of what he was saying, and that’s what his supporters see. At most people get short snippets, where the full incoherence of Trump’s ramblings isn’t as apparent (and the clips are more easily rejected as attempts at “gotcha” moments taken out of context, simple flubs or gaffes, rather than indicators of his mental state).

And talking about how the media’s bite-sized, sane-washed version of Trump’s statements is the only one people see, apparently the Trump administration is aware of it and really wants to keep things that way, because they’re deliberately withholding the transcripts of most of his statements and speeches, particularly the crazier ones:
